A provider–patient relationship is only created when you register as a patient, complete required forms, are accepted by our practice, and participate in a consultation (telehealth or in-person). 4. Online Payments & Stripe
We use Stripe as our third-party payment processor to accept online payments for consultations and services. By submitting your payment information, you authorize us and Stripe to process payments for the services you select. We do not store full credit card numbers on our systems. 5. Prepayment, Cancellations, and Refunds
You may be required to make payment prior to a consultation or service. Unless otherwise stated in writing, prepaid fees are non-refundable if you fail to attend your appointment or cancel without adequate notice as defined by our office policies. Any refund or credit is at the discretion of the practice, subject to applicable laws.

6. Telehealth Services
If you receive telehealth services, you acknowledge that telehealth involves the use of electronic communications and may have limitations, including technical failures. You agree to be physically located in a state where our provider is licensed at the time of the telehealth visit. Our providers may determine that telehealth is not appropriate and may recommend an in-person visit instead.
